Comparison of Operation Methods for Benign Thyroid Diseases: Effectiveness of Gasless Endoscopic Thyroidectomy Via Anterior Chest Wall Approach

Abstract

|
Purpose: A permanent transverse surgical scar is an unavoidable problem after conventional thyroidectomy. Endoscopic thyroidectomy performed via an anterior chest approach leaves no scarring and so provides an excellent cosmetic result. The purpose of this retrospective review was to assess the effectiveness of endoscopic thyroidectomy compared with other thyroidectomy options.
Methods: From February 2004 to February 2006, a total of 75 patients with benign thyroid disease underwent thyroidectomy. Thirty-one patients underwent endoscopic thyroidectomy by an anterior chest wall approach with the gasless system and new retractor, 20 patients underwent MIOT (Minimal invasive open thyroidectomy), and the remaining 24 patients underwent COT (Conventional open thyroidectomy).
Results: We divided the 75 procedures into three groups and compared the mean operating time, the mean blood loss, total amount of post-operative drainage, length of hospital stay, mass size and complications. When compared to conventional thyroidectomy and minimal invasive open thyroidectomy, patients who underwent endoscopic thyroidectomy had a significantly faster return to normal activity (p£¼0.05) but a longer operation time (p£¼0.05). These results indicate that, although there are some challenges to overcome including the long operating time, gasless endoscopic thyroid surgery could become a standard procedure in benign thyroid diseases.
Conclusion: To manage benign thyroid disease, endoscopic thyroidectomy via an anterior chest wall approach is feasible and safe and provides promising cosmetic results. We believe that this approach may play an important role in the management of benign thyroid diseases in the near future.
